Surgery is not necessary for circumcision. Whether circumcision is needed depends on whether there are recurrent infections, difficulty urinating, etc. The main treatments for excessive foreskin include keeping it clean, medication, and circumcision.

    1. Keep it clean

Routine cleaning can reduce the probability of infection caused by excessive foreskin. Gently wash the inside and outside of the foreskin with warm water every day, remove smegma and dry it in time. When there is mild redness and swelling, you can use normal saline to rinse and avoid using irritating lotions. After cleaning, keep the perineum dry and breathable, and choose loose cotton underwear.

    2. Drug treatment

Drug intervention is required when recurrent balanitis occurs. For bacterial infections, erythromycin ointment or mupirocin ointment can be applied topically as directed by your doctor. ; For fungal infections, clotrimazole cream is recommended. When there is obvious edema in the acute phase, desonide cream can be combined with short-term treatment to relieve inflammation. It is forbidden to forcibly turn up the foreskin during medication.

    3. Circumcision

Surgery is required when there is urinary obstruction, scarring phimosis, or precancerous lesions. Traditional circumcision and stapler surgery are common procedures, and the operation time is usually controlled at 20-40 minutes. Pressure dressing is required for 24-48 hours after surgery, and cefaclor dispersible tablets are used to prevent infection. It will take 2-3 weeks for complete healing, and strenuous exercise should be avoided during this period.

    4. Expansion correction

Non-surgical treatments for simple phimosis in children can be tried. Using corticosteroid ointment and progressive manual dilation under the guidance of a doctor, training twice a day for 4-8 weeks can improve foreskin stenosis. This method is ineffective for adult cicatricial phimosis, and improper operation may cause foreskin tearing.

    5. Observation and follow-up

Asymptomatic physiological hyperphimosis can be left alone for the time being. As the penis develops in pre-pubescent children, some cases of excessive foreskin may naturally resolve. It is recommended to review every 6-12 months, focusing on the foreskin retraction ability, urethral orifice shape and secretions. If you experience pain during urination or foreskin incarceration, you need to seek medical attention immediately.

Daily cleaning should be avoided to damage the skin barrier, and the bathing water temperature should not exceed 40℃. Pay attention to limiting spicy and irritating foods in your diet, and properly supplement vitamin B to enhance skin resistance. Parents of teenage patients should instruct them on the correct cleaning methods and should not forcefully turn up the loosened foreskin. During the postoperative recovery period, the dressing needs to be changed according to the doctor's instructions, and if abnormal bleeding or fever occurs, return to the doctor promptly.
