It is a common phenomenon that the size of the testicles on both sides of a man is inconsistent. In most cases, the difference is within 1 cm, which is within the normal physiological range. Testicular size asymmetry may be related to congenital development differences, varicocele, orchitis, testicular hydrocele, testicular tumors and other factors.
1. Congenital developmental differences:
About 85% of men have mild bilateral testicular asymmetry, with the right side usually slightly larger than the left. This difference lasts from adolescence to adulthood and does not affect reproductive function and sex hormone secretion. Congenital developmental differences do not require special treatment, just regular observation.
2. Varicocele:
Left varicocele can lead to delayed testicular development on the ipsilateral side, which is manifested as a reduction of more than 20% in the volume of the affected testicle, which may be accompanied by a feeling of scrotal swelling. Mild cases can be alleviated by scrotal support, while high spermatic vein ligation should be considered for moderate to severe cases.
3. Sequelae of orchitis:
Testicular atrophy may remain after treatment of viral orchitis complications such as mumps or bacterial epididymo-orchitis. Antiviral or antibiotic treatment is required in the acute stage, and testicular spermatogenic function needs to be evaluated in severe cases.
4. Testicular hydrocele:
Abnormal accumulation of fluid in the intrathecal cavity will cause the scrotum on the affected side to significantly enlarge, and the light transmittance test will be positive. A small amount of effusion may heal spontaneously in infants and young children, but if it continues to increase in adults, a tunica vaginalis inversion surgery is required. The difference in testicular size will improve after surgery.
5. Signs of testicular tumors:
Painless swelling of the testicles requires alertness to germ cell tumors, which are often accompanied by hardening and heaviness of the testicles. Testicular ultrasound can identify the nature of the tumor. After diagnosis, radical resection or radiotherapy and chemotherapy are required according to the pathological type.
It is recommended to perform a testicular self-examination once a month by gently rolling the testicles with your hands after a warm bath to feel for changes in size, shape and texture. Avoid riding or sitting for long periods of time on a daily basis and choose breathable cotton underwear. If you find that the testicular volume difference exceeds 30%, accompanied by pain or induration, you should seek immediate medical attention from a urology department. Maintaining moderate exercise such as jogging and swimming can help improve blood circulation in the scrotum. In the diet, pay attention to supplement zinc, oysters, nuts, etc. and vitamin E dark vegetables, and avoid excessive intake of foods with high estrogen content.
